At this time a PayPal window will open, and you will have the option to edit the quantity of cases you'd like to purchase. A PayPal account is not required. PayPal accepts Visa, MasterCard, American Express, or Discover.

Once finished, click the "Proceed to Checkout" button and follow the directions to complete your order.

Submit a Purchase Order

If your Facility has a standard "Purchase Order" Form, you may complete that purchase order detailing the following:

    1. Billing Address
    2. Shipping Address
    3. Product Description
    4. Product Number
    5. Quantity of Cases Being Ordered
    6. Contact Information for the Authorized Personnel Placing the Order
    7. Signature of Authorized Personnel

Or, GT has a standard Order Form available for completion.
